[TYPO3-media] Refactor t3lib_extFileFunctions: newfolder + newfile

Hi,

FYI, I have pushed two patches recently to the Review. Let me know if you have any comments on that. 
There is nothing really new, just carried on the work keeping the style. Though, you will notice 
some additional Unit Test for this t3lib_extFileFunctions.
