[TYPO3-doc] DocBook: first sample manual

I looks good to me. I got it rendered :)
Style looks ugly, but I guess this is not yet a topic. If I use NS xls, I 
get the following error:
> Element title in namespace 'http://docbook.org/ns/docbook' encountered in 
screenshot, but no template matches.

Also the image images/callouts/1.png is never created. (Program listing)
