[Typo3-doc] Anatomy of an extension

Ok - i'll work on the basic thing over the next short while and then we 
can see how to link it to the other docs.

btw: do we have a main TYPO3 doxygen somewhere? Would be really, really 
nice to jump to pages in that. I can sort out the doxygen script, if it 
helps.
