that's how I understand it:
an extension is the whole package which can contain plugins and modules.
a plugin is a FrontEnd Plugin or content element (example tt_news) 
a module is a backend module (example: extension manager)

an extension can contain more than one plugin (example: newloginbox) or more
than one module (example: templavoila)

there are also some extensions that have neither a plugin nor a module f.e.:
clean_db or different_linklayout
