What TYPO3 version are you using? What are the settings you have in the
CONSTANT EDITOR for the following constants:

styles.content.imgtext.alttextSplit
styles.content.imgtext.alttextSplitToken
styles.content.imgtext.emptyTitleHandling
styles.content.imgtext.titleInLink
styles.content.imgtext.titleInLinkAndImg

And is the Text with image being rendered correctly already? Are you
seeing a set of <DL>'s to output the images?
