ever heard of "sticky keys"?
press your shift key five times ;)
...wow! assistive technology!...thats just the simplest solution for 
this problem if no alternative keyboard is available.

further those technologies exist:
Thumb keys: One or more extra keys are added in reach of the thumb.
Multiple state keys: A three state keyboard uses keys which can be 
pushed up, down, or not at all. This gives 243 combinations for one hand.
Additional finger keys: extra row, above or below the base row. This is 
effectively the same as having one multiple state key per finger.

AFAIR this is called "Chordic Keyboard"
