Without any further information I'd first do:

- clear all caches in the Install Tool (this is more thorough that clear 
all caches in the backend)
- check php.ini settings for any opcode cache. If the option to check 
changed files is disabled (or set to a long time period) PHP won't see 
any changes (new files in installed extension, changes in cached 
ext_tables/localconf, etcetera) and these kind of error will continue to 
appear until you manually clear the opcode cache
