maybe it's about absRefPrefix or baseUrl?

maybe the extension always uses relative pathes which is ok if you 
always work in root, which is done if you disable realurl.

with realurl you change the path for the browser, and if the browser 
wants an image with the relative path of fileadmin/img/image.jpg it 
appends it to the 'current path' - which reolves to 
domain/sitepath/fileadmin/img/image.jpg
