[TYPO3-english] ExtDirect security token errors after refresh login

Hi All,

After a login has been refreshed (with the refresh login box) I keep getting 
ExtDirect security token errors. There are many issues about this in forge 
but they all state this has been fixed with the release of 4.5.0.
However I am experiencing this since at least 4.5.11 up till 4.5.18.

Right now the only remedy is to refresh the entire backend, which is of 
course a pity when you have a record with changes opened.

Are any others also epxeriencing this? And if it is particular for my 
installation how can I track the root cause down? The error message itself 
is a bit sparse with information.
