Hi Tom,

Yes - you will need an HTML template. You can build one on your own (i 
always do) or modify an existing one. You don't need a specific tool (i 
always use a plain text editor, like emacs). And of course about 50% of 
a website is about styling... so CSS is part and parcel of the enterprise.

You don't need TV to integate the HTML into Typo3. Maybe it's easier for 
a beginner to use automaketemplate to load & parse the HTML template - 
but this depends on taste and the way work is divded up amongst people 
(if at all).
What goes into Setup wholly depends on your needs - a dynamic menu 
almost always is a good idea though.
