[TYPO3-english] Templatable form

Hi all,

I'd like to use Powermail but I need very special form layout. AFAIK 
this cannot be achived with Powermail as it has only templates for 
fields, but not the whole form, where I could set layout of the form fields.

Am I wrong and is this possible? If so, how?

If not, is there any other extension that you would recommend that would 
allow me to create custom form layout and then send the form values via 
e-mail?

I'm using 4.5.
