[TYPO3-english] Trying to use DAM in BE displays 404 Error Page

I am running TYPO3 4.5.3, DAM 1.2.2, PHP 5.2.15, Linux, Apache server 
api is cgi/fastcgi, register globals are off

Server:Apache mod_fcgid/2.3.6 mod_auth_passthrough/2.1 mod_bwlimited/ 
1.4 FrontPage/ 5.0.2.2635

I have the DAM catedit and index along with realURL.

When ever I try to make a folder, rename a file, delete a file or move a 
file my 404 error page is displayed instead of the proper backend form.

I have narrowed it down to any function using the mod_cmd URL's.

example:

http://www.mydomain.com/typo3conf/ext/dam/mod_cmd/index.php?CMD=tx_dam_cmd_foldernew&vC=7f3f265151&folder=%2Fhome%2Ffredhumm%2Fpublic_html%2Ffileadmin%2Fuser_upload%2Fgalleries%2F&returnUrl=http%3A%2F%2Fwww.mydomain.com%2Ftypo3conf%2Fext%2Fdam%2Fmod_file%2Findex.php%3F%26SLCMD%5BSELECT%5D%5BtxdamFolder%5D%5B%252Fhome%252Ffredhumm%252Fpublic_html%252Ffileadmin%252Fuser_upload%252Fgalleries%252F%5D%3D1

This never happened with v 4.4.x of TYPO3 and the DAM function URL's 
look exactly the same. I have three TYPO3 sites on the same server with 
exactly the same problem.

I am wondering if someone might be able to help me figure out if I have 
a configuration issue.
