We use two different approches.
Either PDFgenerator which takes HTML (4) input and renders it as good as 
it can.
The results do vary though.

Most of the time we use FPDF with a custom made parser.
So TYPO3 generates XML and our parser generates the output we need from it.
This is more effort, but the results are by far better than 
autogenerated stuff.
