[TYPO3-english] Formhandler failed submission

Greetings,

I've been receiving sporadic reports of failed form submissions using formhandler on our site. I have successfully recreated the issue but have not been able to pinpoint the cause.

In all cases the POST goes through and registers in the apache log as a 200 but the page just sits there as though the submit never happened. Hitting submit a second time usually gets the form to successfully submit.

No errors of required fields not being filled out are reported.

This is all running on  typo 4.4.5,  formhandler 0.9.10.

Can anyone suggest a method of further investigation?
