[TYPO3] indexed search target

hi,
I'm just a newbie and i'm having troulbes understanding how to define
indexed search target filetypes...
manual say to use boolean *item_type* (0 for TYPO3 pages, 1 for external
files).
Means I cannot search for both at the same time?

Manual also say to look into *class.indexer.php* file, but it seem to be a
"drastic" solution to me!

Isn't it possible to do that using Setup? if yes, please, how?
