[TYPO3] Problems with fe login when switching T3-4.0.4 -> T3-4.1.2

It's a bit of a long shot, but in your description of your upgrade
process above, you don't mention updating the database--did you run
the Compare option in the Install Tool's "Database Analyser" section?
