I highly recommend Retrospectiva.

It's open source and ruby on rails based - very simple to set up and modify if you've worked with rails before. As a client-facing tickets/management app, nothing beats it for usability and getting the job done. 

I highly recommend Lighthouse as a paid service - it's friendly with email and looks great. Your clients can email tickets and get a great overview about what is happening with the project.

I have found the following Extension in TER[1]. Should be useful if you only
want a form in TYPO3 to report Bugs.
