[TYPO3] Front end user reg - what am I missing

I could swear I've been through all the options tenteen times but can't 
see what I'm missing.

On my creat registration page, when the visitor clicks the create 
account button, the page just refreshes.  It retains all of the entered 
information, but doesn't go anywhere.

Are there any common gotcha's?  I don't see any in the group.
