[TYPO3] TMailform nulti_recipient error

When building a form with pil_mailform with the Dynamic Recipient option 
selected the select menu is not appearing on the form produced. All 
other fields are there.

On the template I have:

<div class="formr">
	<span class="label">What is the subject of your email?</span>
	<span class="formw"><select 
name="tx_pilmailform_pi1[select][multi_recipient]">###MULTI_RECIPIENT_OPTIONS###</select></span>
</div>

And in the Dynamic Recipient box I have:
   Please Select
   Information Request; AnEmailAddress
   Question about an order; AnotherEmailAddress
   Become a Dealer; AnotherEmailAddress
   Other; AnotherEmailAddress

The Multi-recipient option is selected.

Any suggestions?
