[TYPO3] Check and update global reference index - Not showing up in DB Check module.

Hello everyone,

I have installed many typo3 4.0.2 instances.  One of my upgrades though 
has a really wierd thing.  I have upgraded other instances on this 
server and "Check and update global reference index" shows up in the DB 
Check module.  However in this latest one the menu for the reference 
index does not show up.  All the other options in the DB Check are there 
and work.  I even tried going directly to the link 
(typo3/sysext/lowlevel/dbint/index.php?SET[function]=refindex) but it 
gives me a "Filereference, 
"/var/www/html/typo3/ext/lowlevel/dbint/locallang.xml", not found!".

Doubled checked the source and config files and they are identical to 
the instances that are working as far as I can tell.  The locallang.xml 
is in the same spot.

I have uninstalled and reinstalled the db-check extention and it did not 
change anything.

Anybody encounter this or have any ideas?  It would be appreciated.
