I'd try the following:

1. Upgrade TV to the current version 1.1.1 (0.4.0 is quite old at this
point...). You can do this from the extension manager in the BE,

2. Completely reload the BE frameset once the new version of TV is installed,

3. If the error persists, try the problem-creating steps in a browser
like firefox where you can get /meaningful/ line numbers in the JS
errors (the line numbers in the JS errors in IE are often as good as
meaningless).
