th_mailformplus does something like this, but the database table must  
be part of the TYPO3 database.

At T3CON, a very impressive extension called dre_formmaker was  
demonstrated. It could probably solve your problem. It is yet  
experimental and has no manual, but a stable version with manual was  
promised within 2-3 weeks.

And of course, there are many other ways to achieve what you want, as  
usual with TYPO3, but I can't just list them all. :-)
