[TYPO3] TYPO3 4.0.2 on IIS/MS SQL: subpages don't appear in backend tree...

Do you have + on the right of the page at level 1?

You uncheck option to disable classic page module while installing 
templavoila... Click on templavoila in the ext manager and check this 
option. Then refresh BE.
