[TYPO3] including php scripts into T3 v4.0?

Hi,

I have a problem concerning a php script I want to include in T3 site.
I have simple php script that prompts user for some information, takes
that information and put it into a mysql database. After clicking a
submit button user is redirected to another page. This page only checks
whether the user entered all information or not.

So, basicaly this a simple php script. I try to inserted on a pge as a
plain html, but it doesn't work. One page shows the form an works fine,
but the other shows php code.

Does anyone knows how to import this script and make it work? I need a
simple and fast solution.
