Look at the source code of HTMLAREA browse_links.php
I tested with an installation, which uses HTMLAREA and it worked there.
You might have misunderstood the purpose of title.
It is just title for mouseover (<a href="..." title="..." ...)
You can't never make link using the link button without painting an area 
- in HTMLAREA the button doesn't do anything if any area has not been 
painted (TinyMCE should do the same).
