[TYPO3] Typo3 4.0 Default Login Form failure

Typo3 4.0 database is freshly installed. I just set up page User and Login
following the way they had been setup in QuickStart which version is 3.8.1.

Hi Jennifer,

	When you upgraded from 3.8.1 to 4.0, have you done the following??

1. Go to install tool and upgrade to 4.0
2. Go to install tool --> Database --> update all table
