[Typo3] determine if current menu item is the first one

Hi Matt.
You could either use "optionSplit", or create 2 seperate menus, 1 with the 
1st item, and the 2nd with the rest of the itemes, by using the "begin" and 
"maxItems" properties of TMENU/GMENU.
