[Typo3] Syntax for creating another level in the ###SUB_L2MENU###

Greetings.

I have looked for documentation on this very easy menu system.  I want it to
be simple.  Yet I just want to add one more level to the side right menu and
I can¹t seem to figure it out. I am using an html template throughout with 2
cols.  The markers look like this   <!-- ###SUB_L2MENU### start    -->
                        <table width="100%" border="0" cellspacing="2"
cellpadding="2">
                          <!-- ###SUB_L2MENU_ALLITEMS### start    -->
                          <!-- ###SUB_L2MENU_ITEM### start    -->
                          <tr>

   then my simple menu links

  <!-- ###SUB_L2MENU_ITEM### end    -->
<!-- ###SUB_L2MENU_ITEM_ACT### end    -->


I feel I have tried everything. Perhaps it is something in the setup?
