[Typo3] Creating a basic extension

Hi everybody  

I follow meticulously the tutotial "Creating a basic extension"

Everything work fine until I decide to use my new plugin in regular page :

    Neither the First Name input  nor the Image field show-up in the 
form !....

For those who had already work on this tutorial, did you get this kind 
of problem ?
If yes what was the cause ?

My version of typo is the 3.7.
