[Typo3] Forms/ checkbox question

Hello,

I'm running typo 3.7 on redhat 3.0as server.

I've got an existing form here:

Which, I'm trying to redo, and get working in typo. I've got it here:

I've got it working fine, but I'm stumped on the checkboxes. How do I
have one label, and multiple checkboxes? Here's the form code I've got
so far:

*Teacher's name: | *Teachers_name=input,40

*Home Phone Number: | *Home_Phone_Number=input,40
