if you always have a heading on top of your content, you could add a 
special marker for it in your design-template.

The marker could then be filled like this:

marks.headline = COA
marks.headline {
   10 = TEXT
   10  {
     field = title
     #required = 1
     #wrap = <h1>|</h1>
   }
}

Hope I got your question right.
