in between i've got a solution without the need of conf.php and 
ExtensionManagementUtility::addModule().

It seems to be possible to just use ExtensionUtility::registerModule() 
twice, once for the main module and once again for the sub module:

The only thing that is still missing is the possibility to change the 
module position in a "nice" way. I still have do insert the new item 
manually in $TBE_MODULES.
