Both ... it works the same way as in 4.x

The extension manager fetches the mirrors.xml.gz, then the extensions.xml.gz 
(both prefixed with the uid of the repository record - 1).
Then it unpacks the the extensions.xml.gz file and imports the content into 
the database.
