He is not responsible for it because nothing has to be written. TYPO3 guaranties that reference index will be ok if you use TYPO3 API. However if you use MySQL triggers or direct SQL to alter fields, TYPO3 is bypassed and built-in updates of references is broken.

Well, the topic is too broad to discuss here... :( If you do not see it as important, no one can force you to use it. This is a free world. However the quality of your work will be lower than it can be.
