You should re-chck your setup. All of your examples above and any
variant I can create return a 404 page with proper response header for
me.

Relevant install tool settings:

[FE][pageNotFound_handling] = /index.php?id=54
[FE][pageNotFound_handling_statheader] = HTTP/1.0 404 Not Found
