What's the problem?

If you're end-user in a hosted enviroment then of of course you have to 
update the extensions YOU want in typo3conf/ext.

OTOH if your the admin of such an enviroment (and want a global upgrade) 
you'll be able to deal with this fact easily (you have access to the file 
system).
