I found out that db_list.php doesn't pass the id to alt_doc.php.

That means that alt_doc.php has no idea in which directory it is used, if you
come from db_list.php. So you have nearly no chance to to do something special
for a directory in ext_tables.php. In this case. Well, you could in fact do a
hack be extracting the last id out out of the return url.

typo3/alt_doc.php?returnUrl=db_list.php%3Fid%3D6%26

As far as you build your own list plugin you can simply pass the id to alt_doc.php.
