I also had some problems with the EM. I couldn't install extension when I changed a configuration parameter in the install dialog. F.e. installing tt_news:
- Import extension
- klick to install
- install dialog appears
- I changed any of the configuration values
- pressing "update"
- nothing happened, extension wasn't installed, no db-tables where created

I only worked if I left the configuration values untouched on first install and changed them after the extension allready was installed. Had this problem with a couple of extensions on two servers (local dev and customer).
