I think this would be misleading. The user might think this is an error
he has to fix, while a content element without a heading is perfectly
normal and valid.

True, my mistake!

I don't know what skip/version you are using, but in "default TYPO3"
(TYPO3 4.0 with the "new" skin) it looks like that:

So next to the icon is the content type, not the header. The header is
in the "content area" as a bold first line, if present, and no bold
line, if not present. This looks fine for me.
