[Typo3-dev] how to create an individual search-form in an extension

Hi,

I need some advice or a documentation how to create an individual 
searchform within my own new extension. The framework is done with 
kickstarter.

The form should look like http://www.molinvest.de/suche_go.php .

How do I set up this form?
How can I use it within my extension?
How can I search within my tables?
How can I make it using the ListView made by the kickstarter?
