I think the language handling (HMENU special=language and content_fallback) 
has some issues.

I have never been able to get what I wanted without TS setup (TCE) tricks 
(hide ALL pages when not translated) and the content fallback simply didn't 
work for me.

Also an issue: name,flag and iso code of default language (and all other 
stuff that might get added to the sys_language table). It might have been a 
good idea to use 0 as default uid, but I think it is a problem now as we 
have no record for the default language.
