well a combination of a clean `n valid HTML template without layout 
information, one or more example style sheets and some configuration 
options via TS seem to be the best solution to me.

I often heard complaints like this recently. So we put the redesign of 
extension HTML templates and writing a chapter for a manual for 
extension developers on the ToDo-list of the accessibility workgroup:

A lot of people have worked on the design-templates of different 
extensions so I would suggest to set up a workgroup for this and 
communicate via the accessibilty newsgroup.
