IMHO changing the position of the first two lines doesn't make much 
sense. Either print the <?xml ...> at the very top, or leave it out 
completely.

But why not introduce an option like
     config.altDoctype = <!DOCTYPE my doctype definition>

Wouldn't that solve even more problems?
