What are the features (besides the alt attribute)?

I don't see any, and Robert doesn't even want alt attributes for image 
w/text conetnt elements.

So without real DBAL, meaningful accessability and complete UTF-8 
support I would not make it a 4.0.

Yes but it's been anounced for ages. I have a very bad feel for projects 
that increase the major release number just like this.
