Just to add my 2cents I'm still an alt/title tag being in core promoter. 
  Although it does add a couple of fields to tt_content so does creating 
an extension and that's a bit of overhead too.

But I think it's fair to say that alt tags for images are as important 
now as width and height attributes and images shouldn't be without them.
