If you use worker, you might run into problems because mod_php is not
threadsafe. In this case replace the apache2-mpm-worker package with
apache2-mpm-prefork.

Try to upgrade to latest version (0.9.5.2 atm)
0.9.5 ships a lot of bugfixes.

Since you spend only 16M for eA, it is probable that stuff gets cached
on disk. Make sure you have enough free disk space left in
/var/cache/eaccelerator and increase the amount of memory
eaccelerator.shm_size="32"

If this still happens after upgrading eA, please open a bug at
eaccelerator.net
